Child Booking — Parent / Guardian Consent

I confirm that I am the parent or legal guardian of the child I am booking, or that I have the legal authority to provide consent on their behalf.

I give permission for the child to participate in martial arts training, classes, events and related activities conducted by The M A Connection.

I understand that martial arts and combat sports involve inherent risks, including physical contact, strenuous exercise, falls, takedowns, throws, joint locks, chokes, strikes, use of training equipment and other activities that may result in injury.

I knowingly and voluntarily accept these risks on behalf of the child and consent to their participation.

I confirm that I have disclosed any medical condition, injury, allergy or other information that may reasonably affect the child’s ability to participate safely.

In the event of an emergency, I authorize The M A Connection to arrange appropriate first aid or emergency medical assistance for the child where reasonably necessary.

To the fullest extent permitted by law, I agree to the applicable The M A Connection Waiver, Release & Assumption of Risk Agreement on behalf of the child.

By accepting this waiver electronically, I confirm that the information I have provided is accurate and that I have read, understood and agree to the above.
