Waiver, Release &

I acknowledge that participation in martial arts and combat sports involves inherent, known, and unknown risks, including but not limited to physical contact, intense and strenuous exercise, takedowns, throws, joint locks, chokes, strikes, conditioning drills, and the use of training equipment, mats, and facilities.

I understand that these risks may result in serious injury, permanent disability, disfigurement, emotional injury, illness, paralysis, or death, as well as damage to personal property or the property of others. Injuries may include, but are not limited to, bruises, cuts, bloody noses, broken bones, sprains, dislocations, concussions, cardiovascular events, or other serious injuries.

I further understand that instructors and staff are not infallible and may make errors in judgment; may be unaware of a participant's fitness or abilities; may give inadequate warnings or instructions; and that equipment or facilities may malfunction or present hazards.

I knowingly, voluntarily, and expressly assume all risks, whether foreseen or unforeseen, associated with participation in activities at The MA Connection.

I affirm that my participation is entirely voluntary, and I choose to participate despite the risks involved.

To the fullest extent permitted by law, I hereby release, waive, forever discharge, and covenant not to sue The MA Connection from any and all claims, demands, actions, causes of action, losses, or liabilities, whether arising from negligence or otherwise, that are in any way connected with participation in training, use of equipment or facilities, or presence at The MA Connection.

This release applies to claims made by me, or by anyone acting on my behalf, including my heirs, assigns, personal representatives, and estate.

I agree to indemnify and hold harmless The MA Connection from any claims, damages, losses, liabilities, or legal costs (including solicitor's fees) arising out of my participation or from enforcement of this agreement.

I authorise The MA Connection and any qualified medical personnel to provide emergency medical treatment as deemed necessary in the event of injury or illness. I understand that I am solely responsible for any medical or related costs incurred.

I declare that:

• I am physically and mentally capable of participating safely;
• I will not participate while under the influence of drugs, alcohol, or medication that may impair my ability;
• I have disclosed any medical or physical conditions that could affect my safety, or I voluntarily assume all risks associated with such conditions.

I certify that I have adequate insurance coverage, or I agree to personally bear all costs of injury, illness, or damage.

I acknowledge that training and activities may be photographed, filmed, recorded, or observed by spectators or media.

I grant The MA Connection the irrevocable right to use my name, likeness, image, voice, and appearance in photographs, video, or other media, in print or electronic form, for lawful purposes including marketing, advertising, education, and online content, without compensation.

I, the person signed below, am the parent or legal guardian of this underage member/person. I have read and fully understand this entire Waiver, Release & Assumption of Risk Agreement.

On behalf of the Minor, I consent to their participation in all activities conducted by The MA Connection and knowingly and voluntarily assume all risks associated with such participation.

To the fullest extent permitted by law, I hereby release, waive, forever discharge, and agree to indemnify and hold harmless The MA Connection from any and all claims, demands, actions, or causes of action arising out of or related to the Minor's participation or presence at The MA Connection.

I authorise emergency medical treatment for the Minor if required and accept full financial responsibility for any medical costs incurred.

I confirm that I am the lawful parent or legal guardian of the Minor and have the authority to enter into this agreement on their behalf.

I acknowledge that:

• I have read and fully understand this document;
• I understand that I am giving up substantial legal rights;
• I am legally competent to sign this agreement;
• I sign this waiver freely and voluntarily.

This agreement shall be binding upon me, the Minor (if applicable), and our respective heirs, assigns, and legal representatives.
